Job Description
We are seeking a RN to work Temporary full time, 5 days a week as an Advanced Practice Solutions employee at a Shelter based clinic to support services to clients who are experiencing homelessness. This position will be part of a multi-disciplinary team that provides primary care, care coordination and acute care services to a wide variety of clients in Minneapolis, Minnesota. This assignment is on going and the hours are Monday through Thursday from 8am - 4:30pm except Wed is 5-8pm and then Friday 8am-noon (40 hours a week)
Job Functions (Duties/Responsibilities):
RN is responsible for providing nursing care and coordination for adults, youth, and children who are experiencing homelessness and provide preventive health and acute care services on a walk-in and outreach basis. This role supports the mission to advance the well-being of individuals and families experiencing homelessness by 1) Providing high quality, equitable, and comprehensive health care that recognizes the impact of systemic harm 2) Respecting patient autonomy 3) Emphasizing flexibility: connecting with patients both where and how they need care. The staff nurse job classification may serve a variety of functions including clinic supervisor, float nurse, respite nurse, maternal child health nurse, and street outreach nurse.
The Staff Nurse will provide direct nursing care to patients including triage, assessment, screening and intervention, and will carry out orders of HCH providers to people experiencing homelessness.
- Conduct initial and ongoing health assessments and triage to determine patients physical, social, and behavioral health care needs and to assist the health care team in meeting these needs.
- Deliver skilled nursing care and interventions following RN policies including, but not limited to medication and vaccine administration, phlebotomy, education, standing orders, vital sign and growth monitoring, wound care, and documentation of treatment using patient-centered goal progress and patient responses within the clinics and community spaces.
- Read and interpret patient charts, lab reports, and provider notes. Review labs of assigned patient pool and communicate labs needing follow up to appropriate provider.
- Provide care coordination, referrals, and work with community partners to ensure seamless delivery of care and care transitions, to improve communication among health and human service providers, and to guide clients and communities toward an effective use of resources
- Document data, plans, clinical actions, client progress, response to care and other relevant patient information in Epic to be in compliance and rules of confidentiality.
- May use standing orders to order labs and/or treat certain diseases in accordance with policy.
- Float 2-3 clinics a week (within a 7 minute drive of each other) street outreach team care
- Pediatric skill set/work history/wants to work with pediatric patients
- Comfortable working with the homeless population
- Phlebotomy skills

- Vaccine (IM) administration experience

Need to have:
Clear MN License
Public Health
At least 2 years of experience in a clinic setting
Passion for serving the underserved population
